Create and publish digital forms
Digital forms combine reusable rich content with structured placeholders and a patient signature. Once a version is published, completed documents remain tied to that exact version.
Build the form in Clinic Settings
- Create and edit rich structured content.
- Add clinic branding such as the logo.
- Insert typed placeholders for clinic, patient, date, provider, service and appointment data.
- Add the signature placeholder where the patient is expected to sign.
- Preview the document before publishing.
Patient signature
The current form workflow supports one patient finger-signature field. The signature is captured in the public completion flow and included in the final generated PDF.
This is designed for straightforward clinic forms and consent documents rather than multi-party legal signing workflows.
Multiple signatures, countersignatures, OTP/eID signing and programmable form logic are outside the current form scope.
Published versions are immutable
A draft can be edited until you publish it. Publishing creates the version that patient instances reference. Later template changes create a new version instead of rewriting completed history.
This matters because the signed PDF must remain attributable to the exact content and placeholders the patient saw.
Start from an existing PDF or DOCX
Main Menaela can prepare a draft template from an uploaded PDF or DOCX. The uploaded document is extracted through the normal attachment pipeline, then converted into an editable draft for review before publishing.
Treat the generated draft as a starting point: check formatting, placeholders and the signature location before using it with patients.
