Generating medical notes
The draft combines encounter evidence with the clinic’s presentation rules. Instructions can shape the format, but they do not get to add clinical facts.
Where note facts come from
The current consultation evidence is the basis of the draft. Clinic instructions and de-identified examples guide structure, terminology and style; they are not evidence about the current patient.
Supported note structure
Depending on the encounter, the draft may organize content into sections such as complaint, history, medications, examination, assessment and plan. Sections should reflect what is actually supported rather than being filled for completeness.
Uncertainty belongs in review, not invention
If transcription or clinical meaning is uncertain, the draft can surface that uncertainty for review. The safer behavior is to ask the clinician to resolve it, not to choose the most plausible medical wording.
