Completed patient documents stay with the patient. Signed forms are preserved as PDFs; questionnaires retain structured answers and a readable completed representation.

Signed forms become patient files
When a form is completed and signed, Menaela generates the final PDF and attaches the completed artifact to the patient record. The stored result is tied to the published template version used for that instance.
Using documents through Menaela
Within the signed-in user’s permissions, Main Menaela can list patient documents, retrieve questionnaire responses, return signed-form metadata or a PDF reference, and report completion status.
Completed history is treated as a record, not a draft to rewrite conversationally.
