Forms & questionnaires overview
Forms and questionnaires are first-class patient documents. Clinics build reusable templates, publish stable versions, attach requirements to visits or services, and send one secure packet for completion.

Two document types, two jobs
| Type | Best for | Completed result |
|---|---|---|
| Form | Consent, acknowledgements and documents that need patient details and a signature. | Immutable signed PDF plus completion metadata on the patient. |
| Questionnaire | Structured intake and patient-reported information. | Structured answers plus a readable completed document on the patient. |
The document lifecycle
- Create a draft template in Clinic Settings or ask Main Menaela to prepare one.
- Review the content and publish a version. Published versions are immutable so completed patient documents stay tied to exactly what was shown.
- Optionally attach the template as a requirement for an initial visit or a specific service.
- Prepare a patient packet. Multiple required items can be grouped behind one secure link.
- Send or share the packet. The public link is opaque and valid for 48 hours.
- Track progress and retrieve the signed form or questionnaire response from the patient record.
Statuses you may see
- Pending — the packet or document exists but has not been completed.
- Opened — the patient has opened the public flow.
- In progress — some work has been saved but completion is not final.
- Completed — the response or signature was successfully finalized.
- Expired — the secure link reached the end of its validity window.
- Cancelled — staff cancelled the packet or requirement instance.
- Superseded — a newer packet replaced the previous one.
Patient-facing rendering
Patient documents support both RTL and LTR rendering so Hebrew and other clinic languages can use the same document workflow without forcing one visual direction.
